Job Description:
Oversees the team's utilization of technology and adherence to standard work to develop and implement effective discharge plans in collaboration with patients, families, care teams, and payors.
Participates in the hiring, onboarding, education, and retention of staff.
Participates in hospital meetings and initiatives to improve the consumer experience and achieve organizational goals.
Coordinates and leads family and care team meetings as needed.
Monitors for process improvement opportunities and participates in performance improvement initiatives and reports quality/risk issues to appropriate parties.
Collaborates and problem solves with outside agencies to facilitate patient transitions to post-acute settings.
Manages daily operations of the care management department in the Inpatient,
Observation, and Emergency Department settings. Collaborates with the management team to create and implement strategies to decrease length of stay, reduce avoidable readmissions, and enhance customer experience.
Leverages dashboards and reports to monitor staff compliance with documentation and regulation requirements.
Actively participates in daily multidisciplinary rounds and ensures staff participation to discuss patient transitions with cross functional teams and address barriers to discharge plans.
Educates and coaches staff to ensure timely and accurate documentation of patient evaluations, discharge plans, and regulatory requirements.
Leads the interdisciplinary team to address discharge barriers and escalates care delays to ensure timely transitions.
Leads staff meetings and ensures staff participation in process improvements, assuring the frontline voice is heard.
Performs other duties as assigned.

Pay Range:
$85,529.67 - $159,089.69
