The VA Hudson Valley HCS is recruiting for two (2) full-time Registered Nurses to serve veterans on the Special Population Home Based Primary Care and Spinal Cord Injury (HBPC & SCI/D) Pact Team. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Previous home care, ER, or ICU experience. AHA BLS Certification and unrestricted, clean drivers license required. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. The HBPC/ SCI/D RN Care Manager serves the veterans on the Special Population HBPC/SCI PACT team. The HBPC SCI/D RN/CM uses the nursing process and evidence-based practice to collaborate with the core Interdisciplinary team, in the Home Based setting. The RN/CM is responsible for comprehensive, longitudinal care in partnership with the veteran/caregiver and coordinates comprehensive health care services consistent with agreed upon goals of care. He/she collaborates with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. The RN/CM provides patient and family health education with a focus on self-management, prevention and wellness, based on the patient's goals. The RN/CM demonstrates leadership by serving as an advocate for patients, team player to colleagues as she/he continues to enhance his/her own and the team's professional growth, development and practice. The RN/CM leads and monitors continuous improvement strategies to improve the team's function, increase proficiency, improve health outcomes and optimize the quality of care. The SCI/D RN/CM's responsibilities include but are not limited to: Using the nursing process and evidence-based practice to collaborate with the Core Team and Expanded Team in developing the patient-driven holistic care plan for life. Utilizing collaborative strategies and creative approaches in the delivery and improvement of holistic care for patients and family members. The CM functions as a member of the interdisciplinary team in the planning and management of care delivery to a complex patient population, supporting the transition and medical needs of patients in the outpatient home setting, decreasing the need for hospitalization and Emergency Room visits when possible. Functioning in a leadership role engaging staff in active problem solving regarding clinical and professional issues, leading to an improvement in care or practice in the HBPC/SCI/D Program. Providing patient and family health education with a focus on self-management, prevention and wellness based on the patient's goals. Demonstrating leadership by serving as an advocate for patients, as a team player to colleagues, to enhance his/her own and the team's professional growth, development and practice. Promoting cost effective use of resources including the use of various telehealth technologies. Coordinating the care of patients with complex medical issues including bowel and bladder needs regardless of location (e.g.SCI outpatient clinic, inpatient, home) in care and/or supply issues, skin care, SCI practices and protocols, referrals, autonomic dysreflexia risks, bowel and bladder issues, etc. Comprehensive care of male/female HBPC/ SCI/D patients which includes prevention and maintenance of physical systemic issues, social, mental health and spiritual domains in each phase of the life cycle. Educates patient and caregiver on proper use of supportive adaptive devices (chairs, lifts, activities of daily living tools, etc.) to promote maximum independence. Assisting with data collection, analyzing, trending, and reporting activities as they pertain to HBPC/SCI/D Program. Assisting with monitoring compliance with Joint Commission and other regulatory agencies, standards and policies. Upholding the VA ICARE core values.
