This Registered Nurse Specialist position is located in Orange Annex, CT location. This position is full-time at 40 hours per week. Duties: 1. Demonstrates competency using the nursing process in providing care for patients. 2. Participates in patient and family education. 3. Directs others who provide care. 4. Assumes responsibility and accountability for individual nursing judgements and actions .5. Acts as a patient advocate. 6. Plans and organizes care based on patient needs and provider competencies to assure safe, efficient, cost effective care. 7. Delegates care in a safe, efficient, and cost-effective manner. Assists patients in identifying and securing appropriate services. Performance Criteria Performs initial nursing assessment and prioritizes needs. Develops plan of care and patient-centered goals based on initial assessment and evaluates patients progress towards goals. Demonstrates knowledge and skills necessary to provide care appropriate to the age of patients served. Adult(a) Accepts adults' chosen life-styles and assists with necessary adjustments relating to health. Recognizes adults' values and function of independence and ability to be self-sustaining in life. Supports change as necessary for health.(b) Assists patients to plan for anticipated changes in life, to recognize the risk factors related to health, and to focus on strengths rather than weaknesses. Geriatric(a) Assists patients to keep physically and socially active and to maintain peer group interactions.(b) Assists patients to cope with loss, e.g., hearing, eyesight, death of loved one.(c) Assists patients with self-care as required, and with maintaining as much independence as possible. Evaluates patient response to care. Documents observations, patient progress toward goals. Follows policies and procedures. Recognizes obvious signs and symptoms and changes in patient's needs. Reports appropriately to Nurse Manager/designee or provider. Follows ANA Nursing Practice Standards. Assesses basic patient/family/significant other education needs. Provides explanations and written information to patients/family. Reviews discharge instructions with patients/family. Documents teaching activities. Follows the ANA Code of Ethics Develops awareness of ethical/legal issues in nursing practice. Fulfills work commitments. Safeguards confidential information. Safeguards information according to the Privacy Act and Cyber security Principles. Verbalizes knowledge of and initiates appropriate action in emergencies. Utilizes equipment and supplies in a safe, cost- effective and efficient manner. Reports unsafe conditions, practices promptly. Practices standard precautions. Adheres to safety and health standards, regulations, and work practices. Identifies Environment of Care issues and seeks resolution. Participates in precepting new employees (RN, LPN, NA, HT) and validates unit-specific competency completion.
