The Southern Arizona VA Health Care System has an immediate need for (1) one Nurse (PACT -Homeless - Mental Health) to serve in the Mental Health Clinic as part of a Patient Aligned Care Team for the Mental Health Service. This position is on the Homeless team and reports directly to the Clinical Nurse Manager of Triage. As a critical component of the Homeless Team you will contribute positively to the Mental Health needs of Americas' Veterans. Responsibilities: Use approved Primary Care Nurse Triage system to triage patients who walk into the clinic, to manage their symptoms as warranted. Assess patients for any mental distress exhibited through actions or verbal cues and involve mental health practitioners as needed in a timely manner. Guide seriously mentally ill patients and give them tools to better manage their disease conditions within their capabilities. Review TRM Call Log Reporter daily as needed to assure capture of all triage patients calling into the TRM Triage system for any needed follow up. Review Daily discharge list in VSSC as needed to capture timely follow up appointments with the PCP for CHF, COPD and pneumonia patients. Call and follow up with patients who have left the Emergency Department AMA or without being seen. Work Schedule: Full-Time 40 hours per week, Monday - Friday, 07:30 a.m. - 04:00 p.m. or 08:00 a.m. - 04:30 p.m. Financial Disclosure Report: Not required
