This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The Home Based Primary Care (HBPC) Program provides primary care to the most vulnerable home-bound veteran population. The SFVA HBPC covers both rural and urban areas of California within San Mateo, San Francisco, Marin, Sonoma, Mendocino, and Humboldt counties. The RN provides nursing assessment, treatment and coordination of medical care within the veteran's home setting in conjunction with the interdisciplinary HBPC team, home care agencies, and consultations with other VA departments and programs for the geriatric veterans of all ethnicities and gender. Major duties and responsibilities include, but are not limited to: Delivering direct nursing care in the home setting. Developing and coordinating the patient's plan of care to include ongoing assessment and re-assessment, care planning, care coordination, monitoring, and evaluating clinical outcomes. Interacting, coordinating and advocating for the patient/caregiver with other relevant parties across a continuum of care, beyond a single episode of care, ensuring seamless transitions. Providing individualized education and support to the patient and caregiver to optimize clinical outcomes and support quality of life decisions. Providing intensive care management. Linking Veterans with resources. Coordinating resources for comprehensive care to avoid duplication of services and streamline staff interaction and services with the Veteran. Participating in the development of the individual plan of care at interdisciplinary care meetings for assigned panel of patients.
