Valor Point (9A) is located within the Community Living Center is a 40-bed unit consisting of 3 programs, Transitional Care, Geriatric Evaluation Management and Comprehensive Integrated Inpatient Rehabilitation Program (CIIRP). Transitional Care (19 beds) is a short stay (less than 90 day) interdisciplinary program which serves medically and psychiatrically stable veterans and provides nursing and medical care prior to their return to a community setting. This program provides wound care/healing IV therapy/TPN, various orthopedic diagnoses, respiratory care; veteran and family education and veterans waiting title 19 approval and guardianship. Geriatric Evaluation Management (10 beds) is a short stay (less than 30 day) program which provides an interdisciplinary team assessment and plan of care to assist in maintaining the geriatric veteran in the community. Veterans must be age 65 or older, be medically stable but have multiple medical, functional and psycho-social problems that could benefit from a team assessment. The Comprehensive Integrated Inpatient Rehabilitation Program (11 beds) is an acute care inpatient rehabilitation program providing comprehensive care by an interdisciplinary team to maximize function and abilities. This program provides an opportunity for appropriate veterans to improve deficits in mobility, self-care or safety concerns with attainable goals. Length of stay is determined by goals of care. Nursing care provided is veteran focused and uses a team model. The patient population is dynamic and complex providing opportunities for nursing staff to experience a vast amount of diagnosis and hone assessment skills. Major Duties include but not limited to: Personal patient care including bathing, feeding, grooming and mobilizing with demonstration of skills which reflect the needs of the older adult. Appropriate support duties i.e., individualized patient care, proper implementation of policies and procedures, use of good body mechanics, and assist in application and operation of specialized functional equipment. Support duties for proper environmental maintenance such as Identification of potential safety hazards. Support duties for treatment services such as specimen collection. Provides Veteran centric care according to the individualized care plan. Work Schedule: Full Time - NOC shift (1130pm-800am) - Holiday and Weekend Rotations Financial Disclosure Report: Not required
