This position is in response to the COVID 19 Pandemic. The Primary Care COVID RN will serve as facility champion and coordinates local PACT implementation, training, communication, outreach and sustainability of the PACT model. The coordinator plans, organizes, directs and evaluates the PACT COVID programs and reports related actions/activities to facility, VISN and national leadership. Duties include, but not limited to: Provides a full range of nursing care to employees utilizing the nursing process to guide employee care, organizational processes and/or systems improving outcomes. Completes initial assessments and reassessments of employee conditions and response to treatment and other clinical findings; initiates follow-up as needed. Assesses employee's learning needs, develops and implements a plan to meet needs, and instructs employees in self-care and treatment measures. Promotes continuity of care through developing an individualized employee interdisciplinary plan of care in collaboration with other specialty areas as appropriate for the condition. Coordinates Occupational Health programs and demonstrates clinical excellence in management of groups receiving education. Monitors the administration of prescribed therapies using appropriate judgment. Initiates referrals to other disciplines and/or services as appropriate. Accurately documents in the employee health record employee findings, assessments, and care provided. Applies problem-solving skills leading to an improvement in employee care. Supports organizational structures and systems that ensure the provision of effective employee health education. Coordinates employee health education initiatives using clinical knowledge of the disease process and principles of adult learning and evaluates new materials based upon population assessment criteria. Demonstrates leadership in Occupational Health quality improvement activities. Identifies opportunities to improve employee care through monitoring, analyzing and evaluation of in Occupational Health outcomes. Applies creative approaches for change in practice based on Occupational Health quality improvement outcomes. Mobilizes appropriate resources in response to situations that may have the potential to negatively impact employee outcomes. Uses a body of research to validate and/or offer changes to Occupational Health practice. Validates practice based on current literature/research and identifies individual employee clinical problems or issues that need investigation. Assists with implementation of research based on literature as it relates to Occupational Health clinical practice, problem, or issue. Work Schedule: 0730-1600 Monday through Friday with a possibility of weekend coverage needed. NOTE: Shifts and scheduled workdays may be subject to change due to needs of the facility for broader coverage.
