The incumbent practice as a Registered Nurse (RN) in Home Based Primary Care (HBPC), a department of the Rehabilitation and Long Term Care (RLTC) Division located in Vancouver, WA. HBPC provides home care and community services for Veterans who have a number of chronic medical conditions or require post-surgical care. Patient populations include male and female, adult and elderly, frail patients. HBPC Nurse provides home health care services but does not provide infusion therapy, home health aide, chore, or hospice care services. The HBPC Nurse staff provides medication management, injections, wound care, anticoagulation and glucose monitoring, blood draws, specimen collection, safety evaluations and assessments, and patient education. The HBPC Nurse is accountable for a primary caseload, for patient assessments, patient/family education, and coordination of the therapeutic regimen in the home setting. The HBPC RN demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Work Schedule: Monday - Friday, 7:30 am - 4:00 pm or Monday - Friday, 8:00 am - 14:30 pm Financial Disclosure Report: Not required
