The Outreach Nurse provides nursing care in the healthcare system within the scope of practice defined by the Nurse Practice Act and regulatory standards. The /Outreach Nurse assures safety in practice by maintaining a realization of own individual level of knowledge and experience and assumes responsibility for advancing knowledge of self and others of the patient care team by communicating with and utilizing resource persons and programs available. Responsible for consistently setting and evaluating priorities through appropriate interventions with measurable outcomes toward improvement; utilizes the nursing process to integrate all aspects of the provision of care and individualizes the plan of care to address a specific patient's needs. Additionally this cope of practice is extended to the community setting as it relates to the outreach element of nursing and engagement and retention of healthcare services.
BA/BS/BSN preferred. Additional experience required based upon specific department needs.
Current RN NJ Licensure
BLS Healthcare Provider Current
Experience and knowledge of HIV is preferred
Experience working with urban populations of multiple ethnic and cultural backgrounds
Good oral and written communication skills
